How to create an action plan to fulfil a learning contract

What is an action plan and what does it entail? How do you create an action plan that is executable in line with your objective?
An action plan could be described as a statement of intent or a map to fulfil a set of objective (s) in any given circumstance. It could be a form of statement to meet the need of different stakeholders in a learning environment as evidence for developing and completing a learning process. For example, a teacher on teaching practice could develop an action plan to satisfy different learning demand between the student, mentor or the institutions involved in the  learning contract.
In developing an action plan you have to answer these important questions:
What do you need to learn - you must identify your learning needs?
Which needs can you realistically work on at this point in time - Your Target
What can you do to learn the things you have set out as your target - Action plan
What evidence will you gather to help check the progress you are making - Review
In conclusion action planning, towards meeting a learning contract will consist of four main areas;
Identify your learning needs, setting targets action plan and review.
The following can be shown as sources of evidence:
Assessment you designed
Notes of meetings with mentor
Your log
Work book
Lecture note
Essays
Notes made from lessons you observed
Book reviews
Lessons plans
Notes fro meetings, workshops or seminars attended or
Notes from teacher who observed your work
